Set in the 1950s, the story follows Hellboy (Jack Kesy) as he teams up with rookie B.P.R.D. agent Bobbie Jo Song (Adeline Rudolph) in the Appalachian Mountains. After a monster-related train accident leaves them stranded, they discover a small community plagued by witches and a demonic entity known as the Crooked Man. This malevolent figure has a mysterious connection to Hellboy’s past and has returned to collect souls for the devil.
As they delve deeper, Hellboy and his allies confront supernatural horrors, including traumatic visions and dark rituals. The Crooked Man's power is linked to the town's mines, and a climactic battle ensues in an abandoned mansion and underground tunnels, culminating in a confrontation that tests their resolve and strength.
