Set five months after Thaddeus "Thunderbolt" Ross (Harrison Ford) is elected President of the United States, the story follows Sam Wilson as he navigates his new role as Captain America. Alongside Joaquin Torres, the new Falcon, Wilson is dispatched to Oaxaca, Mexico, to recover stolen classified items from the mercenary group Serpent, led by Sidewinder. While they succeed in retrieving the items, Sidewinder escapes, and the mission sets off a chain of events that escalate into a global crisis.
President Ross invites Wilson and Torres to a summit at the White House, proposing a treaty to govern the mining and distribution of a newly discovered metal, adamantium, found on "Celestial Island"—a landmass formed from the emergence of the Celestial Tiamut in the Indian Ocean. During the summit, the song "Mr. Blue" triggers several attendees, including super-soldier Isaiah Bradley, to attack Ross and other dignitaries. The attackers later claim no memory of their actions, indicating a deeper conspiracy at play.
Investigations lead Wilson and Torres to uncover that Dr. Samuel Sterns, also known as the Leader, is behind the mind-control attacks. Sterns, who gained advanced intelligence after exposure to Bruce Banner's blood, uses technology and the song "Mr. Blue" to manipulate minds. His ultimate goal is to discredit Ross and incite global conflict over adamantium.
